How to Avoid Tax Underpayment Penalties

I am concerning about Tax Underpayment Penalties.  I found a article in a website, but I am still not sure how to do it. It is too late to ask employer to withdraw some more money from income, and Stock broker could not do it either. Could anybody give me a clear explanation how to it, which form (1040EX?) and before when I have to file to avoid the penality.

Thanks for the answer.
I read a post from Da Qian forum about underpayment tax. It seems that I need to file a 1040EX form before Jan 15, I could not find it anymore.
