Sounds like my English is too bad...
Words such as "Bought, Sold, Covered, Got out..." means "did" when that bar is closed or near close.
Set-up is my view for good entry either long or short, but not necessary to trade or just not traded or price not get filled.
Trading Notes is not after market review or analysis. It is like taking notes or record data when you do testing in the lab, in my view.

My purpose to post my notes is to support Cobra and learn each other. But it seems I should not post this anymore.
